The alfalfa sprout E. coli and Salmonella outbreak 2026 has been linked to 55 illnesses across 15 US states. Federal health authorities have advised consumers not to eat alfalfa sprouts sold under the Calco or Everything Sprouts brands. Restaurants, grocery stores, distributors and other food businesses should not sell or serve the affected sprouts.[1,2]

The outbreak is unusual because investigators have identified three strains of Shiga toxin-producing Escherichia coli, also called STEC, alongside Salmonella Agona. Forty-six people have been infected with E. coli, seven with Salmonella and two with both pathogens. Four patients have been hospitalised, although no deaths had been reported in the latest official update.[1]

Anyone who has the affected alfalfa sprouts should discard them or return them to the place of purchase. Washing recalled sprouts is not sufficient. Containers, refrigerator shelves, cutting boards and other surfaces that may have touched the sprouts should be cleaned thoroughly with hot soapy water to reduce cross-contamination.[1,2]

Alfalfa Sprout Outbreak 2026: Latest Case Count

The FDA and CDC investigation has connected the illnesses to alfalfa sprouts distributed by Everything Sprouts, LLC, based in Minneapolis, Minnesota. Illnesses began between May 31 and August 8, 2026. Among 34 patients interviewed about what they ate before becoming ill, 26—or 76%—reported eating alfalfa sprouts.[1]

The known illnesses have been reported in Florida, Indiana, Iowa, Kansas, Michigan, Minnesota, New Hampshire, New York, North Carolina, North Dakota, Pennsylvania, South Carolina, South Dakota, Washington and Wisconsin. Because the investigation is continuing, the case count, distribution information and product guidance may change.[1]

The E. coli strains identified in the investigation are O26:H11, O103:H25 and O168:H8. The Salmonella strain is Salmonella Agona. Traceback evidence collected from grocery stores and restaurants identified alfalfa sprouts distributed by Everything Sprouts as a source of illness, and the FDA began an inspection and sample collection at the company on August 19.[1]

Which Alfalfa Sprout Brands Should You Avoid?

The current federal warning applies specifically to Calco-brand and Everything Sprouts-brand alfalfa sprouts, including products supplied through wholesale channels or repackaged by retailers. It does not mean that every product sold by these brands or every type of sprout in the market is included.[1,2]

Everything Sprouts subsequently issued a voluntary recall notice covering alfalfa sprouts distributed from May 27, 2026, onward to certain third-party distributors and retail grocery stores in Minnesota and Wisconsin. The company’s notice describes Everything Sprouts alfalfa sprouts in a five-ounce plastic container with a lid.[3]

As published on August 21, the company notice refers to “following lot numbers” but does not display a usable numeric lot code. Consumers should therefore follow the broader FDA and CDC brand-level warning rather than relying on a lot number to decide whether a package is safe.[1–3]

A restaurant sandwich, salad, wrap or buffet item may contain sprouts that were removed from their original packaging. Customers should ask the food business to confirm the brand or supplier rather than assuming that unlabelled sprouts are unaffected. The FDA has specifically instructed retailers that repackaged affected sprouts to remove them from sale.[1]

Why Raw Alfalfa Sprouts Can Carry E. coli and Salmonella

Alfalfa sprouts are grown from seeds in warm, moist and humid conditions. Unfortunately, the same environment that allows seeds to germinate also allows small numbers of E. coli, Salmonella and other bacteria to multiply rapidly. Contamination may already be present in or on the seed, making it possible for bacteria to reach high levels even when sprouts are produced under apparently sanitary conditions.[8]

Unlike the outside of a firm fruit or vegetable, bacteria associated with germinating sprouts may not be limited to an easily cleaned external surface. Rinsing can reduce some surface contamination, but the FDA states that washing does not eliminate bacteria. Thorough cooking generally reduces the risk from non-recalled sprouts, but a recalled product should be discarded rather than washed or cooked for consumption.[1,8]

Cross-contamination is also possible. Sprouts may transfer bacteria to refrigerator drawers, reusable containers, knives, cutting boards, salad bowls or ready-to-eat food. After discarding affected sprouts, wash hands and clean surfaces and utensils with hot soapy water. Dishwasher-safe items can be cleaned in a dishwasher.[1,2]

E. coli Symptoms After Eating Alfalfa Sprouts

The E. coli organisms involved in this outbreak are Shiga toxin-producing strains. STEC infection commonly causes severe abdominal cramps, diarrhoea that may become bloody and vomiting. Fever may be absent or relatively mild in some patients, so the absence of a high temperature should not be taken as proof that the illness is harmless.[2]

Symptoms can vary from mild diarrhoea to serious illness. Children, older people, pregnant patients, immunocompromised individuals and people with underlying kidney or chronic medical conditions require particularly careful assessment, although severe complications can occur in previously healthy people as well.[2,5]

Hemolytic uremic syndrome is the major emergency

A small proportion of people with STEC infection can develop hemolytic uremic syndrome, or HUS. This is a serious complication in which red blood cells and platelets are damaged and kidney function may deteriorate. HUS can lead to kidney failure, long-term health problems or death and requires hospital treatment.[5]

Emergency warning signs include passing very little urine or no urine, blood in the urine, unusual paleness inside the lower eyelids or cheeks, unexplained bruising, tiny red spots on the skin, marked tiredness, irritability or reduced alertness. Anyone developing these symptoms during or shortly after diarrhoeal illness should go to an emergency department immediately.[5]

Salmonella Symptoms After Eating Alfalfa Sprouts

Salmonella infection typically causes sudden diarrhoea, fever and abdominal cramps. The diarrhoea may occasionally contain blood or mucus. Nausea, vomiting and headache can also occur.[2,6]

Most uncomplicated Salmonella illnesses improve within approximately one week, but significant fluid loss can cause dehydration, particularly in infants and older adults. Some patients continue to experience abnormal bowel habits for several weeks or months after the acute infection has resolved.[6]

In less common cases, Salmonella can spread beyond the intestine into the bloodstream, urine, bones, joints or other organs. Infants, older adults, people with weakened immune systems and people with certain heart, vascular or major joint conditions are at increased risk of invasive disease.[6]

When Should You Contact a Doctor?

Contact a healthcare provider promptly after possible exposure if diarrhoea becomes bloody, diarrhoea continues for more than two days without improving, fever rises above 102°F or approximately 38.9°C, repeated vomiting prevents fluid intake, or signs of dehydration develop. Dehydration may present as a dry mouth, dizziness on standing, intense thirst or reduced urination.[2]

Medical assessment should not be delayed in a young child, older adult, pregnant patient, immunocompromised person, transplant recipient or patient living with kidney disease, cancer, diabetes or another major chronic condition. These groups have a greater risk of serious foodborne illness and complications.[2,8]

Tell the clinician that you may have eaten Calco or Everything Sprouts alfalfa sprouts. Include the approximate date of consumption, where the food was purchased or served and whether other household members are ill. Stool testing may be needed to distinguish STEC from Salmonella because the recommended use of antibiotics is different for the two infections.[4,6]

What Should You Do If You Ate the Recalled Alfalfa Sprouts?

A person who ate the affected sprouts but currently has no symptoms does not automatically require medication. The remaining sprouts should be discarded or returned, contaminated surfaces should be cleaned and the person should watch carefully for gastrointestinal symptoms over the following days.[1,2]

Do not taste the product again to determine whether it is contaminated. E. coli and Salmonella do not necessarily change the smell, appearance or taste of sprouts. A normal-looking package cannot be considered safe when it falls under the public-health warning.

When symptoms appear, begin replacing lost fluids while arranging medical advice. Oral rehydration solution is generally more appropriate than relying only on plain water when diarrhoea or vomiting is substantial because it replaces water and essential electrolytes. Patients who cannot keep fluids down, are becoming weak or are urinating less need urgent assessment.[2,4,6]

Treatment of Outbreak-Associated E. coli

Fluid replacement is the central supportive treatment for most diarrhoeal E. coli infections. However, suspected STEC requires caution because some commonly used diarrhoea treatments may increase the risk of complications.[4]

Do not self-administer loperamide or another medicine intended to stop diarrhoea when stools are bloody, fever is high or STEC infection is suspected. CDC guidance states that anti-diarrhoeal medicines should not be used in STEC because they may increase the chance of HUS.[4]

Antibiotics should also not be started independently for suspected STEC. The CDC advises against using antibiotics for STEC infection because they may increase HUS risk. A clinician should first evaluate the patient, exposure history, symptoms and test results rather than treating all bacterial diarrhoea in the same way.[4]

Treatment of Salmonella Infection

Most otherwise healthy people with uncomplicated intestinal Salmonella infection recover with fluid and electrolyte replacement alone. In these patients, antibiotics generally do not shorten the duration of fever or diarrhoea.[6]

Antibiotics may nevertheless be necessary for severe diarrhoea, bloodstream infection, infection outside the intestine or patients at increased risk of invasive disease. This includes infants, immunocompromised patients and some older adults or people with significant vascular, heart or joint disease. When an antibiotic is indicated, culture and antimicrobial-susceptibility results can help the clinician select an appropriate medicine.[6]

This distinction is particularly important in the 2026 alfalfa sprout outbreak because both STEC and Salmonella are involved. An antibiotic decision that may be appropriate for severe Salmonella infection can be unsafe in confirmed or strongly suspected STEC. Treatment should therefore be guided by a qualified medical professional.

How Ayurveda May Support Complete Recovery After E. coli or Salmonella

Ayurveda can have a meaningful role in supporting digestive recovery, but the stage of illness must be identified correctly. Acute bloody diarrhoea, severe dehydration, high fever, reduced urination or possible HUS should never be managed only with home remedies or self-prescribed Ayurvedic medicines. Emergency investigation, hydration, kidney monitoring and pathogen-specific medical decisions take priority.

Charaka Samhita, Chikitsa Sthana, Chapter 19, Atisara Chikitsa, presents diarrhoea management as stage-specific. It cautions against indiscriminately suppressing loose stools with sangrahi or stambhana—bowel-binding interventions—during the initial ama stage and emphasises the assessment of dehydration, patient strength and the use of suitable light, nourishing liquids.[9] This classical principle has an important modern safety parallel: the CDC also advises against anti-diarrhoeal suppression in suspected STEC infection because of HUS risk.[4,9]

For this reason, Kutaja, Bilva or any other grahi medicine should not be taken simply because diarrhoea is present. Bloody diarrhoea associated with this outbreak is not equivalent to ordinary indigestion. The dominant pathogen, disease stage, hydration status, fever, renal function, age, strength and associated medical conditions must first be evaluated.

A root-directed post-infectious recovery model

Once the active infection and emergency risks have been medically addressed, an individualised Ayurvedic programme may help the patient move toward complete digestive recovery rather than merely observing whether stool frequency has stopped. Charaka Samhita, Chikitsa Sthana, Chapter 15, Grahani Chikitsa, describes the central role of Agni in digestion, metabolism and assimilation and provides a classical framework for restoring stable gastrointestinal function after digestion has been disturbed.[10]

Agni can be understood clinically as the patient’s capacity to digest, absorb and tolerate food without persistent heaviness, bloating, cramping or unstable stools. Grahani refers not only to the intestine but to the functional ability to retain, process and appropriately release food. Bala means physical strength, resilience and the ability to recover after illness.

This restorative approach becomes relevant because recovery from foodborne gastroenteritis does not always end when the infection settles. A 2024 systematic review and meta-analysis involving 47 studies and 28,170 participants found that 14.5% developed post-infectious irritable bowel syndrome and 12.7% developed post-infectious functional dyspepsia. Bacterial gastroenteritis was associated with an 18.3% prevalence of post-infectious IBS in the included evidence.[7]

Current research does not establish that an Ayurvedic formulation eradicates these particular outbreak strains or prevents HUS. The most credible integrative model is one that protects the patient during the acute infection and then addresses weakness, appetite loss, disturbed digestion and lingering post-infectious bowel instability in a personalised manner.

Advice for Readers in the UK, Canada, Australia and Singapore

The 55 confirmed illnesses and 15-state distribution described in the current alert relate to the United States. The warning should not be interpreted as evidence that every alfalfa sprout sold in the United Kingdom, Canada, Australia or Singapore is contaminated.[1]

Nevertheless, consumers outside the United States should check the brand name, grower and country of origin when purchasing imported or repackaged sprouts. Restaurants and retailers may remove sprouts from their original containers, so customers can ask the business to identify its supplier. A package labelled Calco or Everything Sprouts should not be consumed while the federal warning remains active.

Consumers should also monitor their national food-safety authority for any local recall or import alert. People who develop bloody diarrhoea, fever, severe abdominal pain, persistent vomiting, reduced urination or dehydration after eating raw sprouts should seek medical advice regardless of the country in which the sprouts were purchased.

How to Reduce the Risk From Alfalfa and Other Raw Sprouts

People at increased risk of severe foodborne illness ,including young children, older adults, pregnant people and those with weakened immune systems are advised by the FDA to avoid raw or lightly cooked sprouts of any kind. This includes alfalfa, clover, radish, onion and mung bean sprouts.[8]

For people who choose to eat sprouts outside an active recall, thorough cooking is safer than eating them raw. A light rinse or brief warming may not kill bacteria throughout the product. Recalled sprouts, however, should always be discarded rather than cooked and eaten.[1,8]

Hand hygiene is also essential when anyone in the household has diarrhoea. Wash hands with soap and water after using the toilet and before preparing food. A person with Salmonella or another infectious diarrhoeal illness should avoid preparing food for others until medically cleared or fully recovered because bacteria may continue to be shed after symptoms improve.[6]
