Difficulty breathing, facial or tongue swelling, fainting and loss of consciousness can indicate anaphylaxis or another life-threatening systemic reaction. Fever, shivering, confusion, a weak pulse, shortness of breath and extreme discomfort can also occur in sepsis. Both situations require rapid assessment rather than observation at home.[6,7]

What is bacterial endotoxin?

Bacterial endotoxin is a heat-stable lipopolysaccharide associated with the outer membrane of certain Gram-negative bacteria. It may be released when bacterial cells are disrupted. When an excessive amount enters the body through an injectable product, it can trigger an intense inflammatory response.[5]

The possible consequences include fever, chills, headache, muscle pain, nausea, vomiting, low blood pressure, circulatory shock and death. Because endotoxin is heat-stable, ordinary assumptions about heating or a product appearing clean do not establish that an injectable solution is safe.[3,5]

Elevated endotoxin does not automatically prove that living bacteria entered the bloodstream. Endotoxin is a bacterial component rather than a living organism. These reported events do not mean that every exposed patient will become critically ill. They do mean that a recent glutathione injection should be disclosed immediately when a patient develops an otherwise unexplained acute illness.

This is not the first FDA warning involving glutathione and endotoxin

In 2019, the FDA investigated adverse reactions among seven patients who had received compounded intravenous glutathione. Within minutes, patients reportedly developed symptoms including nausea, vomiting, lightheadedness, chills and body aches. One experienced low blood pressure and breathing difficulty and was transferred to a hospital.[3]

The FDA received a separate report involving another patient who developed sudden chills, fever and shaking after a glutathione infusion and was admitted to hospital for a possible bloodstream infection. FDA laboratory testing found excessive bacterial endotoxin in all collected glutathione ingredient samples, with some findings reaching five times the calculated appropriate limit.[3]

The 2019 investigation also found that the raw glutathione ingredient had been intended for dietary-supplement use rather than sterile injectable manufacturing. This history demonstrates why an ingredient suitable for swallowing cannot automatically be considered suitable for injection.

What if the injection was received several days ago and the patient feels well?

Feeling well is reassuring, but it is not a reason to continue using a recalled lot.

The patient should stop all further doses, photograph the vial label, record the lot and expiration date, and contact both the dispensing pharmacy and prescribing clinician. The clinician can consider when the exposure occurred, whether any symptoms developed, the administered dose, relevant medical conditions and whether observation or testing is necessary.

A handwritten clinic label without the compounder’s name, lot number, strength and expiration date makes product verification considerably more difficult.

Why does the word “compounded” matter?

A compounded medicine is prepared for a specific clinical need when an appropriate FDA-approved medicine cannot meet that patient’s requirements. Legitimate compounding can be valuable, such as when a patient needs a different dosage form or must avoid a particular ingredient.[4]

However, compounded drugs are not FDA-approved. The FDA does not verify their safety, effectiveness or quality through the standard premarket approval process. Is intravenous glutathione proven for skin lightening or “detox”?

Glutathione is a naturally produced intracellular antioxidant involved in cellular redox balance. That biological role does not automatically establish that repeated high-dose intravenous glutathione provides a proven cosmetic, anti-ageing, immunity-enhancing or general “detoxification” benefit.

A 2016 clinical review found no adequate studies of intravenous glutathione for skin lightening and no evidence establishing the safety of chronic intravenous use.[8] A 2019 systematic review located four small clinical studies of skin-related effects, but those studies evaluated oral or topical glutathione rather than intravenous treatment. The reviewers concluded that the overall evidence for skin whitening remained inconclusive because of inconsistent findings and study-quality limitations.[9]
